maintain the privacy of all client health information.
Any, and all, information (including, but not limited to: name,
phone number,
address, health information,) given to, collected by,
or otherwise obtained by
the Balanced Energy Center, its owner/owner's and/or any employee's
or other
official representative thereof, will be held in strict confidence.
No information will be rented, released, removed, viewed by,
or otherwise
distributed to any third parties without prior written consent
from the client.
We may use or disclose client's health information to a physician
or other
health care provider providing treatment to the client only
with the client's prior
written permission.
We may use or disclose any/all pertinent information when we
are required to
do so by law; or to protect the rights, property, or safety
of the Balanced Energy
Center, it's owner/owner's, or others.
